What this service is

A KIN physiotherapist guiding an older adult with a walker at home KIN

It lets the family talk to a doctor before deciding how to carry on caring for an older relative at home, without having to take someone with limited mobility to a large hospital — the barrier that stops many families from starting at all. These are the same doctors who look after patients at KIN's six rehabilitation centres, so they see the physical condition, mobility, current medicines and the home setting together before advising.

① Rehabilitation medicine — how much function can return, and how to train for it

  • After a stroke — one-sided weakness, an arm that will not lift, a leg that drags — how often to train during the first 3–6 months, when recovery is fastest
  • After surgery — knee or hip replacement, hip fracture, spinal surgery — when weight-bearing can start and which movements to avoid
  • Bed-bound patients — preventing joint stiffness, muscle wasting and pressure sores — positioning and in-bed exercises families can do themselves
  • Swallowing difficulty and frequent choking — whether to thicken food or to have a swallowing assessment by a speech therapist
  • Unsteady walking and fear of falling — balance assessment, then a walking programme set at a level that can actually be achieved
  • Choosing the right equipment — cane, walker, wheelchair or electric bed — which suits the level of help actually needed
  • Planning the training — how many physiotherapy sessions per week, and whether to do them at home or at a centre

② Neurology — the brain, memory and movement

  • Preventing another stroke — reviewing medicines and risk factors: blood pressure, diabetes, cholesterol and irregular heartbeat
  • Forgetfulness, repeated questions, getting lost, misplacing things — normal ageing, dementia, or depression that looks like memory loss
  • Tremor, slow movement, stiffness, shuffling steps — parkinsonism and other movement disorders
  • Numbness and weakness in the hands and feet — peripheral neuropathy, common in people living with diabetes
  • Dizziness and spinning sensations — separating inner-ear causes from blood pressure and neurological causes
  • Evening or night-time confusion and behaviour change — seen in dementia, and worth assessing for other contributing causes
  • Seizures — both newly started and recurring after a period without them

③ Geriatric medicine — several conditions at once and many medicines

  • Several chronic conditions together — diabetes, blood pressure, cholesterol, heart and kidneys — which to bring under control first
  • Taking many medicines — reviewing everything in use for duplication, or medicines causing drowsiness and raising the risk of falls
  • Frequent falls — finding the real cause — medicines, blood pressure dropping on standing, eyesight or balance
  • Poor appetite and unintended weight loss — assessing malnutrition, which is common and often overlooked in older adults
  • Weakness, tiring easily, walking noticeably slower — frailty, which raises the risk of more serious illness
  • Insomnia, chronic constipation, urinary incontinence — problems that affect daily quality of life for both the person and the caregiver
  • Sudden confusion after coming home from hospital — a state that must be told apart from dementia, because the care differs
  • Planning long-term care — care at home, day care, or a residential centre — matched to the level of dependency and what the family can sustain

Frequently asked questions

How much is a consultation, and does it include medicine?

Rehabilitation, neurology and geriatric consultations are THB 1,000 each, the same online or at the clinic, medication not included. If the doctor prescribes, the cost is confirmed first. Online psychiatry is THB 1,500 for 30 minutes or THB 2,500 for one hour.

Can the doctor visit at home?

Doctors do not make home visits. You can consult by video call from home or at the clinic on the doctor's clinic days. The services that do come to your home are registered nurses, physiotherapists and caregivers.

When can I consult online?

The three specialties work by appointment and the team offers the doctor's available times. Online psychiatry runs every day from 16:00 to 19:30, booked at least one day ahead.

Can the family consult on behalf of an older relative?

Family members can consult for guidance, but diagnosis and prescribing require assessing the person themselves. Many families start with the caregiver consulting alone, then book a joint session.

Do we have to use KIN services afterwards?

No. The consultation is a medical opinion. If you then want KIN to continue the care — physiotherapy at home, nurse visits or rehabilitation at a centre — the team will arrange it, but there is no obligation.

When should we not wait for an online consultation?

For sudden symptoms suggesting a stroke — a drooping face, sudden one-sided weakness, slurred speech or loss of consciousness — call 1669 immediately. This service works by appointment and is not an emergency service.
